Background

Welcome to the Hope for Hearts website, a part of a national aortic stenosis education initiative supported by an unrestricted education grant from Edwards Lifesciences.

1 in 8 elderly Australians is affected by aortic stenosis (AS). Many live with the debilitating symptoms of severe aortic stenosis, which impacts quality of life and adds considerable cost to the healthcare system. Your specialist services can help treat many of those suffering, but we need to improve patient access to care. We believe we need to make it easier for patients and general practitioners to find you, so we invite you to be profiled on the Hope for Hearts website and contribute any content that you feel would be valuable to share with your referral colleagues.

The aim of the Hope for Hearts website is to improve the understanding of aortic stenosis and make it easier for referring physicians and patients to connect with specialists and hospitals that may offer help to those suffering. To achieve this, the initial launch site will host:

  • aortic stenosis information and downloadable brochures
  • a physician focused AS masterclass series presented by leading Australian cardiologists and cardiac surgeons, and;
  • an accredited heart centre and specialist locator.

The site can also host notifications of your upcoming education events and video recordings of your previous meetings that referring physicians will be able to access upon sign up and verification.

A comprehensive digital marketing campaign will help drive traffic to the site and encourage general practitioners to subscribe for access to a growing library of education material. The site will also act as an SEO strategy for associated hospital and practice websites.

The Hope for Hearts website has been created by Connect the Docs Pty Ltd (CTD) a medical communication company that specialises in peer to peer medical education and therapy awareness programs. Advisory Panel –
The objective of the Hope for Hearts website is to be a single destination for trustworthy and valuable aortic stenosis information. To ensure the integrity of the site, Hope for Hearts has established an advisory panel made up of multi-disciplinary specialists, a general practitioner and a lawyer specialising in compliance and medical communications.

The Locator and Profiles –
As either an accredited specialist member of a heart team, a referring specialist or as the official representative of an accredited heart program, we would like to invite you and your hospital to be profiled on the site.

The locator will allow general practitioners and patients to quickly locate and contact you through your associated profile links or ‘call to action’ displayed on your profile.

Each profile will be hosted on a separate page and display your logo/photo and title, and the information and active links you enter on this application form (including links to any booking software you subscribe to). You are also invited to include an introductory video and any video content specific to your services which we can host on your profile.

Content –
Hope for Hearts will host a range of cardiology presentations that will be made available to physicians that sign in as verified registered medical practitioners. We aim to develop CPD accredited content to attract the engagement of general practitioners. If you would like us to host video content from any of your educational events, please contact us at [email protected]

Things you need to know –
Updating or removing your profile
You can request that your profile is deleted or updated anytime by emailing us at [email protected]. Once we receive your request, it will take up to 5 working days to remove or update your profile. When updating your profile, you will be required to approve changes via email before the changes are made to the live site.

Your information –
By completing and signing this application form you agree that Connect the Docs can use all the information you supply to us, including, but not limited to, your name, logo, brand, photo, contact details, and web links, to create and host a profile on your behalf on the Hope for Hearts website and all future cardiac-related websites relevant to your services, created by Connect The Docs and to receive the Connect The Docs newsletter. This information will also be used to populate your free profile on *HealthCombined.

*HealthCombined is a dedicated medical networking and communication platform that is scheduled for launch in early 2019. The main objective of HealthCombined is to connect doctors and healthcare organisations on a secure network, allowing you to build referral networks and share information. You will be notified via email when the site is live. You are under no obligation to accept your HealthCombined organisation profile, however we would like to invite you to do so, and also encourage you to invite your VMOs and referral physicians to join and connect with you so you can securely share information and keep your network up to date.

Profile placement –
The search results for the specialist and hospital locator is postcode specific. In the event of multiple profiles matching the user’s search criteria, the following rules will apply to the order of the profiles displayed.

Physician Profiles –
For the first six months, the order of the physician profiles will be on a first in basis. The profiles will be displayed in the order of CTD receiving completed application forms. After this time, CTD, under consultation with the advisory panel, may implement a content contribution weighting to the order of the displayed profiles. It is in the best interest of all those participating in the Hope for Hearts site that the site has an effective SEO strategy. Fresh content is critical, and CTD would like to recognise those specialists participating in content creation.

Hospital Profiles –
Hospitals that backlink to the Hope For Hearts site, as an additional source of cardiac information from the content area of their website will be allocated priority placement.

Profiles will otherwise be displayed in order of CTD receiving the completed application forms.

Profiling terms and conditions –
Connect The Docs reserves the right to update its application form and profiling policy from time to time. Profile subscribers will be given three months notice of changes to the profiling terms and conditions or profiling policy.

We are committed to ensuring that your information is secure. Connect The Docs takes reasonable steps to protect all personal information within our direct control from misuse, interference, loss, unauthorised access, modification or disclosure. In order to prevent unauthorised access or disclosure we use respected hosting services, firewall and other electronic security procedures and managerial procedures to safeguard and secure the information we collect from you. We rely on third party providers such as Google and Mammoth Cloud to store the information you have provided to us securely.We use traffic encryption (https) for all our online services.
We take reasonable steps to hold information securely in electronic or physical form in access-controlled premises or in electronic databases requiring logins and passwords. Our internal procedures may include risk assessment of misuse, interference, loss, and unauthorised access, modification or disclosure of your information, background tracking and records of changes made to your personal information.
Where possible, we request that service providers ensure data is stored in Australia, although that option is not always available. In using our services, you consent to data being sent overseas for the purpose of making best use of the software we use as part of our business.
